Avoid drastic dietary changes. Avoid foods rich in vitamin K. Vitamin K in foods such as leafy vegetables can reduce warfarin efficacy. Avoid grapefruit products. They may interfere with warfarin metabolism and increase INR, increasing the risk of bleeding. Avoid herbs and supplements with anticoagulant/antiplatelet activity. Examples include garlic, ginger, bilberry, danshen, piracetam, and ginkgo biloba. Avoid St. John’s Wort. This drug may reduce warfarin efficacy.
